New Housing Form

BSCIP Case Managers, CMS Nurses and other interested individuals may refer individuals with disabilities who are currently experiencing difficulties obtaining appropriate housing. The Housing Referral form is provided below to facilitate the referral process. Please print and complete the form, then fax or mail it to the FAAST housing coordinator. FAAST's Fax number is: 850-487-2805.
