Vendor Requirements
Please review the following 16 Vendor Service Categories and determine which categories are most applicable to your training, skills and knowledge. Once you have identified your appropriate categories, please forward the required materials to FAAST. All Vendors who wish to work with the FAAST/BSCIP program will be required to meet the requirements listed below. Please register and provide the required material as soon as possible so that FAAST can register you in the system.

Listed below are the meanings of the acronyms used.

ATP-Assistive Technology Practitioner
ATS-Assistive Technology Supplier
OT-Occupational Therapist
PT-Physical Therapist

CDRS-Certified Driving Rehabilitation Specialist

CCC-Certification of Clinical Competency

SLP-Speech & Language Pathologist

NRRTS-National Registry of Rehabilitation Technology Suppliers

DBPR-Department of Business and Professional Regulations

NMEDA-National Mobility Equipment Dealers Association

CRTS-Certified Rehabilitation Technolgy Suppliers

CEAC-Certified Environmental Access Consultant/Contractor 

All FAAST approved vendors must supply the following information:

Complete vendor registration application

Copy of current professional and/or occupational license/certification

Copy of Federal Tax ID Certification

Copy of Professional Liability Insurance

Additional licensing/ certification is required for the following vendor categories:

Adaptive Computer Evaluations

Must be a Rehabilitation Engineer, ATP, OT, SLP, PT or Vocational Specialist.

Complete vendor registration application.

Copy of general liability insurance.

Copy of current Florida occupational license issued by county/city.

Copy of Federal I.D. Tax Certificate.

Adaptive Computer Resellers - Florida

Complete vendor registration application.

Copy of Federal I.D. Tax Certificate.

Copy of current Florida occupational license issued by county/city.

ATS, NRRTS, or CRTS recommended but not necessary.

National Vendors

Complete vendor registration application.

Vocational Specialist/Training

Complete vendor registration application.

Copy of liability insurance.

Copy of current Florida occupational license issued by county/city.

A criminal background check by the Florida Department of Law Enforcement.

Copy of current certification by the Commission on Certification of Work Adjustment and Vocational Evaluation Specialist or Certified Career Assessment Associate.

Augmentative/Alternative Communication Evaluations

Must be an Audiologist or SLP.

Complete vendor registration application.

Copy of general liability insurance.

Copy of current SLP license or Certification in Clinical Competency (CCC) from the Department of Health.

Copy of Federal I.D. Tax Certificate.

ATP License recommended but not necessary.

Augmentative/Alternative Communication Reseller - Florida

Complete vendor registration application.

Copy of Federal I.D. Tax Certificate.

Copy of current Florida occupational license issued by county/city.

Driver Evaluations

Must be an Assistive Technology Practitioner (ATP), Certified Driving Rehabilitation Specialist (CDRS) or Certified Rehabilitation Engineer.

Copy of current Florida occupational license issued by county/city.

Copy of Federal I.D. Tax Certificate.

Resume detailing year’s experience (2 years minimum) in providing driver’s training/education for person with disabilities.

Copy of vehicle operator’s license issued by Florida Department of Highway Safety and Motor Vehicles or equivalent.

Driver Training Specialist

Must be a Certified Driving Rehabilitation Specialist (CDRS).

Copy of current Florida occupational license issued by county/city.

Contractors providing Home Modification evaluations must provide 3 references. References will be checked to see if contractors are experienced in providing Home Modification evaluations for consumers with disabilities.
